Order Form for The DiabeticOptiCarbDiet

The DiabeticOptiCarbDiet (DOCD) files are in MS Word .doc and MS Excel .xls 2002 files, for good compatibility. They can be provided in other formats, but some presentational aspects may differ. (Please enquire using email address at bottom of page). The files are virus-checked before being sent to you.

Licensing. Each DOCD copy gives: a) Healthcare professionals and business users a licence for one location only. (Please ask about multiple locations). b) An individual a licence for the personal use of one person only.

Disclaimer. DOCD is designed for use by primary care medical professionals and Diabetes Educators, to provide printouts to pre-diabetics and Type 2 diabetics not yet on BG control medication. Equally, for use by such diabetic individuals who follow it independently. Other Type 2 diabetics (usually newly or well-controlled) on BG control medication, must get their medical professional's agreement to transfer to DOCD. DOCD is not advised for pregnant women, because they have different dietary needs, or for those with a medical condition requiring specific treatment. Although DOCD only involves healthy balanced eating, it is sold on the condition that nobody connected with it will be liable for damages, of any nature whatsoever, resulting from its use. DOCD users should consult their medical professional if in any doubt about matters of a medical nature.

DOCD costs only GB Pounds 15.00, including delivery and any UK Value Added Tax. That is about US$25 or Euro 20, depending on the actual exchange rate; benefit from the current weakness of the GB Pound. Payment can be by credit card or debit card, or through a PayPal account. o Doing so will open PayPal's secure (https) form. If you already have a PayPal account, you can log into it (on the right) and pay from it. If not, click Continue just above the credit/debit card logos. On the next page, enter your credit or debit card information in the fields provided. Wait about 10 seconds after selecting your "Country", so as to allow any of the other field titles to change appropriately. E.g. UK's "County" becomes USA's "State".
o You will cancel the order if you click "Cancel and return to Roger Grant" before the last page. But you can still return from there to this Order Form, or to DOCD's Home Page – where you can read about participating in DOCD's global cyberresearchproject (in "HOW CAN I BUY DOCD ?"), if you haven't done so already.
o When the transaction is complete, you can print a record of the transaction, and you will also receive an email from PayPal confirmiing it. I will email you the DOCD files – usually the same day.
